SCTIMST MPH & DPH Entrance Exam 2023 Application form

• The application form can be filled from the official website only. 
• The last date to fill the application form will announce soon.
• The candidate has to pay the application fee to submit the application form.
• The fee once paid will not be refunded.
• The candidate has to upload the scanned copy of the images as prescribed by the authority.
• The candidate must take a print of the application form for further process.

Documents required to fill the application form

• A passport size photograph (with white background)
  
• An authentic document to prove age
  
• Caste certificate for SC/ST/OBC(Non-creamy layer)issued by revenue authorities, not below the rank of a Tahsildar (Valid as per Govt. of India norms).
  
• Documents to prove the EWS category, if applicable.
  
• Documents to prove the PWD category, if applicable.
  
• “No Objection Certificate” in case employed
  
• Certificates to prove academic qualifications and achievements (soft copies of all certificates in a single file)
  
• Registration Certificate: Medical (MBBS/MD/MS/DNB)/Nursing.

How to Apply for SCTIMST MPH & DPH Entrance Exam 2023?

Step 1.Registration

• Visit the SCTIMST official website.
• Enter the registration details like:
  • Name of the applicant
  • Date of Birth
  • Email Id
  • Phone number
  • Password

Step 2. Fill the application form

After registration candidate has to fill the application form by providing personal and qualification details like:

• Candidate's name
• Parents name
• Email Id
• Contact No.
• Gender
• Permanent Address.
• Category
• Nationality
• 10th standard passing year
• Marks obtained in 10th standard
• 12th Standard passing year
• Marks obtained in 10+2
• Choose exam center

Step 3. Uploading documents

After filling the application form candidate has to upload the documents as prescribed by the authority.

Step 4. Application Fee payment

The candidate has to pay the registration fee to confirm the submission of the application form. Given below the category wise application fee:

Candidates can pay the application fee via Credit card/Debit card.

Step 5. Submission of the application form

The candidate must save and take print of the filled application form for further process.

The candidate has to bring the following documents on the day of interview along with them.

• Online Payment Receipt.
• Document to prove age.
• Caste certificate for SC/ST/OBC (Non-Creamy Layer) candidates (issued by Revenue authorities not below the rank of Tahsildar).
• ‘No Objection Certificate’ in the case of employed/sponsored applicants.
• Certificates to prove academic qualifications.
• Certificate to prove experience.
• Registration Certificate: For Medical/Nursing graduate
